DERMAL FILLERS (Restylane/Juvederm)The development of dermal fillers has added an exciting new dimension to world of cosmetic medicine. Dermal fillers are materials that can be injected into the skin to fill out a wrinkle or fold or, using advanced techniques, to recreate more youthful and attractive facial contours. The most commonly used dermal fillers today consist of hyaluronic acid, a sugar molecule naturally present within the skin and reproduced in the laboratory for medical use. Unlike older collagen fillers Restylane and Juvederm injections can be carried out without the need for a skin test and the results last significantly longer, with Juvederm lasting up to 1 year in some cases. Before & After Dermal fillers can be used to fill out almost any wrinkle or fold. To achieve the best result however it is necessary to focus not only on lines and wrinkles but also the contour and volume changes that occur at the same time. The patient in the above before and after example had dermal filler injected to the lines around the mouth with the objective to reduce their appearance, rather than to completely eliminate them. This was combined with restoring volume to the cheek area, which overall creates a much more natural and effective rejuvenation effect. The treatment were also combined with botulinum toxin injections in the upper face. The areas which are commonly treated with dermal fillers include the deep nose to mouth lines (nasolabial folds), the mouth corners (oral commisures), lips, cheeks, jawline, tear trough and temples. The key to successful rejuvenation is not about making dramatic changes to any one of these features but to make a series of smaller volumatic changes which create an overall more youthful and attractive appearance. The volume of filler which needs to be injected is determined by a number of factors including your age, the depth of your wrinkles, the degree of volume loss you are currently suffering from and what you ultimately want to achieve. Most people will require 2-3 syringes for their first treatment although fortunately each treatment has a cumulative effect and most patients only require less at subsequent follow up treatments. Dermal filler injections are usually very safe and effective. In general most side effects are relatively minor, short lived and uncommon in experienced hands. To help you understand what can go wrong however, how it would be treated and what the likely final outcome would be we have added a page dealing specifically with this issue. Please read this section fully if you are considering having a dermal filler injection. Alternative treatments to dermal fillers include botulinum toxin injections, chemical peels, fat transfers and surgery. Dermal fillers can be used to fill out almost any wrinkle or fold although to fulfill their true potential in facial rejuvenation it is essential to look beyond only lines and wrinkles and address the important contour changes that occur with ageing. As we get older fat is lost from the face as it is re-distributed around the body whilst at the same time the bony skeleton of the face is remodeled with a general coarsening of the features. Many of these subtle changes in the shape of the face can be reversed by injecting a little filler in the right places - which is where the true artistry of dermal filler injection lies. Some patients ask about the availability of 'permanent' fillers. There are a few permanent fillers available although these products do not age naturally within the skin and can look very odd as you get older. In addition permanent fillers can be associated with a long term risk of complications and require surgery to remove them leaving permanent scarring on the face. As it stands we do not see any permanent fillers as safe enough to offer in our practice.
